Medical Authorization and Emergency Contact

Effective Date: 8 August 2026 Operator: Fastpitch Authority Contact: support@fastpitchauthority.com Completed by the parent or legal guardian of the athlete named in the registration ("the Athlete"). ## 1. Authority to obtain treatment You authorise Fastpitch Authority staff, and any physician, hospital, emergency medical technician or other licensed provider they call upon, to examine and treat the Athlete in the event of injury or illness at one of our events. This authority applies when you cannot be reached in time, or when in the reasonable judgement of our staff or an attending provider delay would put the Athlete at risk. It extends to first aid, transport by ambulance, hospital admission, diagnostic imaging, anaesthetic, and surgery where a licensed provider judges it necessary. You accept financial responsibility for any treatment or transport provided. ## 2. Information you must give us You must complete, and keep current, the following in your account before the Athlete takes part: - Two emergency contacts with telephone numbers, at least one of whom is not travelling with the Athlete - Health insurance carrier and policy number, if any - Known allergies, including to medication, food and insect stings - Current medications, including any carried by the Athlete such as an inhaler or auto-injector - Any medical condition relevant to athletic activity, including asthma, diabetes, epilepsy, cardiac conditions, bleeding disorders, and any condition affecting heat tolerance - Any injury in the last twelve months, and whether she has been cleared to return to full activity - Any concussion in the last twelve months, and the date she was cleared If any of this changes before the event, you must update it. Incomplete or inaccurate information may prevent us from giving appropriate care. ## 3. Medication Our staff do not administer medication. If the Athlete carries emergency medication, she or an adult accompanying her must keep it with her, and you must tell our staff at check-in what it is and where it is. ## 4. Concussion and injury during an event If our staff observe a possible concussion or a significant injury, the Athlete will be removed from all activity for the remainder of the event. She will not be permitted to return that day regardless of how she feels, and no refund is due. We will tell you what we observed and recommend you seek medical assessment. ## 5. Heat Events may take place in high temperatures. Water is provided and rest is permitted at any time. Our staff may withdraw an athlete showing signs of heat illness. You should ensure the Athlete arrives well hydrated and appropriately dressed. ## 6. Records Information given here is used only to care for the Athlete at an event and is held under the Fastpitch Authority Privacy Policy. It is not shown to college coaches, is not published on her profile, and is not used for any other purpose. ## 7. Acknowledgement By typing your name and submitting this form you confirm that you are the parent or legal guardian of the Athlete, that the information you have given is true and complete, and that you authorise treatment as set out above.
